Turning Eighteen Doesn't Come With a Handover Note

For most of her life, Priya's mum was the one who remembered. Which specialist prescribed which medication, when the last blood test came back, what the endocrinologist actually meant by keeping an eye on the numbers. Priya has had type 1 diabetes since she was seven. Her mum sat in every appointment, asked the follow-up questions, and kept the notes in a folder on the kitchen bench.

At nineteen, Priya's paediatric endocrinologist referred her to an adult diabetes clinic. It's a normal part of growing up with a chronic condition in Australia, and clinicians call it a transition of care. What it actually felt like for Priya was walking into a room by herself for the first time and realising she didn't know the answers her mum used to give.

This is the part of chronic illness that doesn't get talked about much. The condition doesn't change when someone turns eighteen. The person responsible for tracking it does.

Priya sat with her mum and set up her own Blooclu account before that first adult appointment. She added her paediatric endocrinologist's notes and a decade of appointment history to her Personal Appointment Log, so the new clinic wasn't starting from a blank file.

She didn't cut her mum out entirely. Priya added her to a Care Circle, so her mum still gets a plain-language update after each appointment, the same kind of update Priya reads herself. Her mum isn't running the file anymore. She's just still allowed to see it, because Priya chose to let her.

The adult clinic doesn't know Priya's history the way the paediatric team did. That gap is usually where continuity breaks down for young adults moving between services. Priya's version of it looked different: a first appointment where she brought her own record instead of arriving empty-handed and hoping someone else remembered for her.

Managing your own health record for the first time doesn't have to start from nothing. If you're old enough to see an adult specialist on your own, you're old enough to set up a Blooclu account and bring your own history with you. Get started for free at blooclu.app/register.
