Three Conditions, Four Providers: How Frank Keeps One Consistent Story

Frank is 71 and lives outside Ballarat. Type 2 diabetes was his first diagnosis, fifteen years ago. Atrial fibrillation came next. Arthritis settled into his right knee two winters back and never really left. None of these conditions cancels the others out, and none of them waits politely while he deals with the rest.

In a typical month, Frank sees his GP, an endocrinologist, a cardiologist, and a physiotherapist. Each one looks after a different part of him. None of them is in the room when the others examine him, and until recently, none of them had much idea what the others had said or changed.

He used to carry a manila folder of printed pathology results to every appointment and still forgot half of what mattered once he sat down. His daughter Susan, who lives forty minutes away, usually heard about medication changes after the fact, in a phone call that started with "so anyway, the cardiologist did something with my tablets."

That's changed since Frank started using Blooclu. His Personal Appointment Log holds a running timeline of every appointment he's had, across every provider, including the physiotherapist who has never touched Blooclu and never will. When the cardiologist adjusts his blood thinner, that visit sits in the same timeline as the endocrinologist's notes from six weeks earlier. Nothing gets left in a folder at home.

Before each appointment, Frank fills out a short pre-visit screening form: what's changed, what's worrying him, which medications he's actually taking versus what's written down somewhere. His physiotherapist reads it two days ahead of his knee review and already knows the cardiologist changed a dosage the week before. She doesn't spend the first ten minutes catching up. She spends them working on his knee.

After each visit, Frank gets a plain-language summary, written for him to read again on the drive home or a week later when he's forgotten the details. It's written for him, in his terms, so he can actually act on it. He shares that summary with Susan through Care Circle, so she can follow what's happening without becoming his unofficial case manager to do it. When she notices something between visits, a bit more swelling in his knee, a day he seemed unsteady, she can add a note there too, before it turns into an emergency.

Frank still has the same four providers he always had. He hasn't traded them for fewer appointments or a simpler condition. What's different is that each one now starts from where the last one left off, and his family can stay close to his care without sitting in on every appointment.

If you're juggling more than one condition and more than one provider, you can set up your own account at blooclu.app/register and start building a record that follows you, not the other way around.
