A new referral arrives. Attached is a letter from the GP, a page of results, and not much else. The patient sits down, and you start reconstructing, again, a history that has already been assembled somewhere, just not for you.

This is a familiar first fifteen minutes for any specialist. A patient managing a chronic condition rarely sees one clinician. There's the GP, maybe two specialists, an allied health practitioner, and each of them holds a piece of the picture. None of them holds all of it.

A record that follows the patient, not the referral
