Most Platforms Stop at the Appointment. We Don't.

A consultation has a clear start and end. You see the patient, you document the visit, you move to the next one. Most tools built for clinics are built around that same window: what happens during the fifteen minutes in the room.
But the appointment isn't where continuity of care actually breaks down. It breaks down before, when a patient shows up without knowing their own history. And after, when what you said gets forgotten, misremembered, or never makes it to the next specialist they see.
The gap nobody's tool was covering
A patient with three specialists and a GP is, in effect, running their own care coordination with no training and no system. When that fails, it shows up in your consult room as a patient who can't tell you what their cardiologist changed, or one who leaves your office and calls back a week later asking what you meant.
