Managing One Diagnosis Across Three Different Doctors
How many times have you explained the same diagnosis to a new doctor this year?
Sarah was diagnosed with type 2 diabetes four years ago. She sees her GP every three months, her endocrinologist twice a year, and a podiatrist whenever her feet need checking. Each of those appointments used to exist on its own. Walking into her GP's office, she'd try to remember what the endocrinologist had changed at the last visit, whether her metformin dose had gone up, and whether her last HbA1c result was good news or something to worry about.
Before her most recent GP visit, she filled out a pre-visit screening form a few days ahead of time. It asked about her current medications, any new symptoms, and anything that had changed since her last appointment. She mentioned that her endocrinologist had increased her metformin dose two weeks earlier and that she'd noticed some mild stomach upset since. That detail reached her GP before she sat down.
During the appointment, her GP already knew about the dose change and the side effect. Instead of spending the first ten minutes reconstructing her recent history, they talked through whether the stomach upset was worth adjusting for. The conversation started where it needed to, not from zero.
After the appointment, Sarah received a plain-language summary of what was discussed and decided, written for her to reread at home. When she saw her podiatrist the following month, she pulled up her Personal Appointment Log, a single timeline that holds every appointment she's had, across every provider, even the ones not yet using Blooclu. Her podiatrist could see the metformin change and the GP's response without Sarah repeating the whole story again.
Managing one condition across multiple providers means the same facts get told over and over unless something holds them together. The Personal Appointment Log is that place. Pre-visit screening forms make sure new information reaches the right doctor before the visit starts. Plain-language summaries mean Sarah doesn't have to rely on memory to carry decisions from one appointment to the next.
None of this replaces her doctors. It just means her history follows her, appointment to appointment, provider to provider.
